Challenges of Revision Rhinoplasty

The etiology of the problem in unsatisfactory rhinoplasty is not always clearly defined by inspection and palpation, as is usually the case with the unoperated nose. Performing endonasal revision surgery may not reveal the cause and thus may not correct the problem. As a result of prior surgery, the anatomy and structural components may be quite different. In addition to the altered anatomy, scarring, thickened tissue, and poor planes of dissection make this second procedure more difficult. Primary surgery has usually removed some of the framework of the nose, leaving less to work with in revision surgery. The patient's psyche is also different. He is now dissatisfied with the results of a surgical procedure rather than just being unhappy with his appearance. Lastly, and possibly most significantly, the patient and the result belong to the last operating surgeon. Therefore, the surgeon contemplating secondary rhinoplasty must be sure he has the means to meet the challenge.
